This is a diagnostic tool using a macro and micro question set that enables certified users to diagnose the root causes of incidents involving human error reliably and quickly.

For each cause and contributing factor identified, it also indicates possible corrective and preventive actions. This tool has been embedded into the incident analysis procedures of leading global corporations who have reported 40-60% reduction in investigation effort and duration.

This is a one-day course for all personnel investigating non-conformances, safety incidents, quality issues and other errors in the workplace.

This program provides the skills required to carry out effective investigations. It includes key information on how to interview people to obtain accurate, relevent information to enable identification of the real root cause, using the Error Investigation and Diagnostic (EI&D) Guide.

Participants are lead through a structured process to determine the type of error and the appropriate corrective action.
